Understanding Venous Disease: Causes & Progression
Venous disease is defined as the impairment of blood flow back toward your heart. Understanding venous health means understanding the complex, interconnected system of vessels that support our lower extremities.
Our legs are comprised of a network of veins that are similar to branches on a tree: they contain large, or major veins and increasingly smaller veins. Oxygenated blood is constantly being pumped from the heart to the rest of our bodies through arteries. It is the job of our veins to carry deoxygenated blood back to the heart.
Healthy veins have delicate internal valves which open and close to assist the return of blood to the heart. Venous insufficiency (also called venous reflux) occurs if these valves become damaged, allowing the backward pooling of blood in the lower extremities. Because of gravity, these weak vessel walls are under tremendous pressure. When blood cannot be properly returned to the heart, it pools over time, leading to chronic heaviness and fatigue, while causing visible varicose veins, surface clusters, and progressive skin changes.
Over time, this increased pressure can cause additional valves to fail down the line. If left untreated, it can result in chronic leg pain, persistent swelling, non-healing wounds, and other systemic health problems. Prevalence

Symptoms & Stages of Venous Disease
Spider Veins
CEAP C1Spider veins are small blue or purple-colored vessels that occur just under the skin but are close enough to be easily seen on the surface. They can cause localized physical discomfort, but because of their prominent appearance, they are frequently a major source of cosmetic self-consciousness during warmer seasons. Clinically evaluated using the universal CEAP scale (where 'C' represents Clinical Severity), surface spider veins sit at stage C1, signaling an early warning phase of venous disease where prompt treatment can halt deeper vascular deterioration.
Varicose Veins
CEAP C2Varicose veins are visible veins in the leg that bulge, twist, or protrude prominently beneath the skin. They are the direct result of advanced venous insufficiency. Classified as stage C2 on the clinical CEAP scale, these breakdowns can cause legs to ache and/or fatigue. Left untreated, individuals often experience a significant change in their overall quality of life.
Swelling
CEAP C3While early stages may only present visual changes, progression into CEAP stage C3 introduces chronic physical swelling (edema), especially around the ankles and lower calves. This accumulation of fluid often worsens by the evening or after long stretches of standing, offering temporary relief only when the limbs are fully elevated. Patients at this stage frequently experience restless leg syndrome, localized itching, or intense muscle cramping.
Continuous swelling compromises skin integrity, placing individuals at high risk for cellulitis—a serious bacterial skin infection requiring antibiotics—along with painful superficial phlebitis clots. Tracking this fluid retention is a key clinical metric used to map appropriate treatment interventions before tissue damage sets in.
Skin Changes
CEAP C4Reaching stage C4 on the CEAP scale indicates that cellular skin changes are actively underway due to chronic blood pooling. As red blood cells break down under high pressure, iron deposits leak directly into surrounding tissues, creating a permanent dark brown, rusty stain on the skin surface. Other advanced manifestations include lipodermosclerosis—which causes underlying tissues to harden and thicken abnormally—and stasis eczema, leaving the leg scale-like, inflamed, and highly fragile.
Leg Ulcers
CEAP C5 - C6Venous stasis ulcers represent the advanced stages of untreated vascular breakdown. When deoxygenated blood remains stagnant in the lower leg for months or years, local cells are deprived of oxygen, causing the skin tissue to slowly deteriorate and open into a chronic, weeping wound. Reaching stage C5 indicates a healed ulcer boundary, while an active, open wound sits at stage C6.
These sores require constant dressing changes, are prone to re-infection, and significantly impair personal mobility. Because they fundamentally cannot heal on their own while the underlying vein pressure remains active, correcting the deeper valve failure is the only path to closing the wound permanently.
Many patients endure painful ulcers for decades under the false assumption that it is simply an inevitable consequence of aging. However, directly treating the underlying valve dysfunction offers exceptional success rates, allowing even long-standing wounds to dry, close, and experience complete recovery.
Other Complications of the Venous System
Superficial Thrombophlebitis
Superficial Venous Thrombophlebitis (SVT) occurs when blood flow becomes highly turbulent and stagnant, forming a localized clot inside a surface vein sitting above the deep muscular layer. While surface clots rarely break loose into the heart or lungs, they spark aggressive, painful inflammation that can incapacitate a patient for several weeks. These inflammatory episodes almost always recur unless the structural venous insufficiency feeding them is mapped and sealed.
Because these issues frequently develop right at the critical intersections where superficial branches join the deep system, precise diagnosis requires a detailed ultrasound vein-valve mapping to evaluate safety and guide treatment accurately.
Deep Vein Thrombosis
Clots that form deep within the interior muscular network are classified as Deep Vein Thrombosis (DVT). This is an immediate, acute, and life-threatening medical emergency. If a deep clot fractures loose, it travels directly through the bloodstream into the lungs, causing a pulmonary embolism. While surface venous reflux is considered an incremental risk factor for DVT, the two conditions are distinct pathological pathways.
